Bridging the Gaps of the Stage 3 Health Information Exchange (HIE) Objective

We are excited to announce that, as part of the first Annual National Care Transitions Awareness (NCTA) Day, we will be hosting a webinar titled “Bridging the Gaps of the Stage 3 Health Information Exchange (HIE) Objective.”

The HIE objective has three measures that must be reported by all eligible providers participating in the Illinois Medicaid Promoting Interoperability program. These measures entail sending, receiving, and incorporating summary of care records electronically when patients are transitioned into our out of the eligible provider’s setting of care. In this webinar, Promoting Interoperability expert Sam Ross will provide an overview of each measure. The learning objectives include:

1. Understanding national goals for safe and effective care transitions and coordination

2. Defining transitions of care and summary of care records for the Medicaid PI Program

3. Understanding the numerator/denominator/exclusion criteria of each HIE measure
